My theory: John saw Sherlock fall, but not land due to a) being hit by the bike and b) Sherlock positioning him during the phone conversation so John's view of the ground was obscured by a low building. Also there was a conveniently placed rubbish truck that drove off after the fall which I think is where Sherlock landed with the body on the floor being a corpse provided by Molly. John was dazed after his fall when he finally fought his way through the crowd and just assumed it was Sherlock

It was not altogether clear why Moriarty felt the need to put a gun in his mouth and pull the trigger. I am led to believe that if you do that, you tend to remove most of your head in the process - i don't think i'll test that though.

Presumably Mycroft would be the one called upon to identify the body, so he'd have to be involved.

Ah, Jim's head did seem remarkably in one piece. and his hair looked odd.

Anyway, if this turns out to be another case of "oh hell, i've killed off the main character! how the hell can i write myself out of this" stuff like the frankly unbelievable mess that was the end of the last Doctor Who season I shall give up on modern TV and spend my days watching Star Cops.

If i recall...Sherlock Holmes returned in a story called "The Empty House"
You are correct. And Doyle never explained what happened.
