What I really don't understand is how any of the management still have their jobs. Their job is to manage the staff below them, to carry out the wishes of those above. If people were dying unnecessarily they should have been sorting out the failures in their own departments, anything else is gross misconduct surely? If a fault is found with certain staff, their line managers should be fired for not fixing it (and their managers etc.). Why else do managers get more money, if not for the responsibility?

I've known several staff up there over the years, and concerns have been there for at least the last 6. You know its time to worry when an A&E nurse tells you to call them if you are in trouble rather than an ambulance
